If you choose to have your baby at Endeavor Health, we make it easy for you to pre-register for your hospital stay.

Pre-register with MyChart

Pre-registration should be completed as early as possible in your pregnancy. Use MyChart* to pre-register for your baby’s birth. Don’t have a MyChart account yet? Sign-up today

*MyChart OB pre-registration is available to patients whose physician offices are with Endeavor Health Medical Group (EHMG).

Other online option

If your obstetrician is not an Endeavor Health Medical Group provider (this applies to any independent provider, including Duly providers), you can still complete your pre-registration online by clicking on one of the links below. You will also need to include copies of your photo ID and health insurance card (front and back). Please fill out the form for the hospital where you’ll be giving birth.

Pre-register at Elmhurst Hospital

The average length of stay for a mom having a vaginal delivery is two nights. A C-section typically requires 3-4 nights in the hospital. Check with your insurance company to make sure your OB/GYN, your baby's doctor (pediatrician or family practitioner), and the hospital where you’d like to deliver (Edward or Elmhurst hospitals) are all covered by your insurance plan.

Once you arrive at the hospital, you will be asked to sign a “Consent for Treatment” form and we’ll ask to see your ID and insurance cards.
