Vaccine reporting for non-employees

Vaccine reporting to include influenza (flu) vaccine, COVID-19 primary series, and other system-wide required vaccines.

You will need to know your 6-digit user ID to complete the following form. Your reporting manager at NS-EEH can provide you with this information if you don't know it.

Note: Per the new systemwide vaccination policy, all non-employees who started on or after 9-15-2022 are required to also log proof of MMR, Varicella (Chicken Pox), and TDAP (if in a clinical role) on the Vaccine Reporting form.

There are 3 ways for non-employees to comply with our flu vaccination program: 

1. Get your flu vaccination at an EEH flu clinic.
Edward-Elmhurst Health provides free flu vaccinations. View the flu clinic schedule overview flyer for non-employees.

Please fill out this consent form and bring it with you when you receive your flu vaccine.

2. Get your flu vaccination outside of EEH. 
Click here to submit your proof of flu vaccine and COVID-19 primary series.
You can also use this form to capture the data from another facility or you can use the facility's form. 

3. Submit a flu vaccination exemption.
Non-employed medical staff and volunteers flu vaccine exemption request deadline was 11/1/22. If you filed an exemption, you will receive an email notification of the determination within the next few weeks.

All other non-employees including residents, interns, students, and contract/agency staff can submit an approved exemption that has been vetted by their employer or school.
