Important MyChart® update for Edward-Elmhurst Health and Duly Health and Care patients

Beginning on April 9, 2022, all new Duly Health and Care medical record information will no longer be accessible through your Edward-Elmhurst Health MyChart account. After this date, you will need two separate MyChart accounts, an Edward-Elmhurst Health account and a Duly Health and Care account, to access your respective medical records.

Note: You will still be able to see past Duly test results in your Edward-Elmhurst Health MyChart account.

For Edward-Elmhurst Health medical record information:
Continue to use the Edward-Elmhurst Health MyChart website or mobile app, MyEEHealth™, to access your health information at Edward-Elmhurst Health hospitals, physician offices and outpatient facilities. Your username and password will stay the same.

For Duly Health and Care medical record and physician order information:
Go to the Duly Health and Care MyChart website or mobile app to access your health information at Duly, or to set up a new account.

After April 9, if a Duly patient chooses to have their lab or radiology test done at Edward-Elmhurst Health, they will need to have their physician fax over an order or bring a paper copy of their order to their test.

Link your MyChart accounts:
Log in to your Edward-Elmhurst Health MyChart account. In “Your Menu” under “Sharing,” go to “Link My Accounts”. When you link accounts, you can use your Edward-Elmhurst Health MyChart account to view health information from your Duly MyChart account and other organizations where you and your family have medical records. You will be notified when this is available for Duly.
