For admission to Linden Oaks Behavioral Health, you will need to complete an initial behavioral health assessment to determine the most appropriate level of treatment. The assessment is conducted by one of our specially-trained counselors and is a free, completely confidential assessment of your symptoms so we can better understand your needs.

Emergency walk-in assessments are available 24/7 at the Linden Oaks Naperville Main Campus, located at 852 S. West Street in Naperville.
When you arrive at a Linden Oaks inpatient or outpatient site for an assessment, you will be asked to review and complete the following forms:

Other items you will need to bring with you to your assessment include:
- Driver’s license or identification card
- Insurance card
- Current list of medications (to be completed on your Intake Information Form)
- Names and contact information for existing providers, including your primary care physician, psychiatrist, and/or therapist (to be completed on your Intake Information Form)
