Jumpstart Your Health Diabetes Prevention Program

If you have prediabetes or other risk factors for type 2 diabetes, it’s time to take charge of your health. The Jumpstart Your Health program can help you make lasting changes to reduce your risk of type 2 diabetes.

In the Jumpstart Your Health program, you receive:

  • 16 weekly group sessions followed by 6 monthly sessions to help you maintain healthy lifestyle changes
  • A trained lifestyle coach to guide and encourage you
  • A Center for Disease Control approved curriculum
  • The skills you need to lose weight, be more physically active, and manage stress
  • Support from other participants with the same goals as you

Program Schedule

Phase 1

MONTH 1 - 2: Weekly Group Education Sessions
Topics Covered:

  • Self-monitoring weight and food intake
  • Eating less
  • Healthy eating
  • Physical activity
  • Balancing calorie intake and output

MONTH 3 - 6: Bimonthly Group Education Sessions
Topics Covered:

  • Problem solving
  • Healthy eating out
  • Reversing negative thoughts
  • Social cues
  • Managing stress
  • Staying motivated

At each class, your weight will be taken in private. At months 6 and 12, a Hemoglobin A1C test will be repeated.

Phase 2

MONTH 7 - 12: Monthly Group Education Sessions
Topics Covered:

  • Handling holidays, vacations and special occasions
  • More volume, fewer calories
  • Stress and time management
  • Preventing relapse
  • Healthy cooking tips
  • Making active choices

At each class, your weight will be taken in private. At months 6 and 12, a Hemoglobin A1C test will be repeated.
