Medicare limited coverage
If it is likely that Medicare will not cover a test, your patient will need to complete an Advanced Beneficiary Notice (ABN) before the test is ordered. The ABN Form serves as a notice that there is reason to believe Medicare will determine the test to be medically unnecessary, and therefore, will not pay for it. A copy of the signed ABN Form must be submitted with the test order.

When ordering genetic tests such as Maternal Serum QUAD Screen, AFP Amniotic Fluid, AFP Serum, Chromosome Studies, Products of Conception or Cystic Fibrosis, we ask that you complete a Genetic Test Form and forward it with the specimen.
A reflexed test is a secondary test performed when a primary test result is outside of established result parameters. These secondary tests are performed to confirm the primary results or enhance the patient's clinical picture.
For some tests, performing the reflexed test is considered common practice and does not require additional orders from the physician. Other reflexed tests, though they may be clinically relevant, are not automatically performed unless specifically requested by the physician, usually at the time of the initial test order.

Couriers may be dispatched for STAT specimen pickup by calling Client Services at 331-221-4422. Additional charges are applied to requests for a STAT specimen pickup.

We’ll work closely with you through every step of the process, including specimen collection. You can expect accurate identification, quality results and timely reporting as your patient's specimens are monitored through our computerized tracking system from pick-up to delivery.

Storage conditions and courier transport

Specimens or cultures classified as "Category A" Infectious Substance are regulated by state and local government agencies and may only be transported if certain conditions for packaging and transport are met.
Specimens or cultures classified as "Category B" Biological Specimens are regulated by state and local government agencies and may only be transported if certain conditions for packaging and transport are met.

To request a site survey for computer equipment or service for EMH Reference Laboratory equipment already placed in your office, please call or email Client Services at 331-221-4422 and ask for the LIS Outreach Analyst.
Please note that the provision of equipment and supplies must be in accordance with federal and state laws and regulations. We provide equipment and supplies to our clients at no charge or below fair market value, but they must be used exclusively in conjunction with laboratory services provided by EMH Reference Laboratory.
Results of tests are posted as soon as they’re available. You can sort results by patient name, ID number, date of service, date of birth, sex or accession number.

EMH Reference Laboratory is among the largest full-service clinical laboratory facilities in the Chicagoland area. We are affiliated with Elmhurst Hospital, part of the Edward-Elmhurst Healthcare system, and have a partnership with ARUP Laboratory and Mayo Medical Laboratory.
We are accredited by the College of American Pathologists and The Joint Commission. Our medical and technical staff is certified in the field of pathology and laboratory medicine, including coagulation, molecular diagnostics, immunology, surgical pathology, cytology, clinical hematology, chemistry, microbiology, molecular virology and flow cytometry.

Associated Pathology Consultants (APC) provides guidance and expertise to our lab. APC is a service-oriented physician practice committed to providing high-quality pathology and medical laboratory services.

Our Client Service Center is available Monday through Saturday to answer questions and assist you with accessing all of our laboratory client services, including:

- Requisitions and supplies
- Technical support
- Arrange for medical consultation
