How do I make an appointment?
Select the location and physician you’d like to see, and call the appropriate number. If you need immediate care, let us know and you will be promptly scheduled. Our goal is to provide timely medical treatment.
How do I reach the different EMPCA offices?
When you call the number at the EMPCA location of your choice, a receptionist will ask for your name, telephone number and the nature of your call. You may also be asked to provide your birth date, any known allergies, and current insurance information. If your call is not urgent, your physician will return your call as soon as possible or after office hours.
What if I have to cancel my appointment?
We understand that things come up and you may need to cancel your appointment. All we ask is that you notify us at least 24 hours in advance. When you call, provide the name of your physician and the date and time of your appointment. You may reschedule your appointment at that time.
How do I get a copy of my medical records?
Your medical record is confidential, and information will not be disclosed without your written consent. When you request a copy of your records, you’ll need to sign a release form and pay for the cost of duplication. Once your request is complete, your copies will be ready within five working days. To request a copy of the release form, contact our Medical Records department at 331-221-9076 or download the form. Emergency situations are expedited.

This is an Elmhurst Memorial Hospital outpatient clinic. It is important to note that you or your insurance carrier will receive two bills for today’s visit that, combined, represent the total cost of your visit. One bill will be for the professional or clinical fees from the visit and the other bill will be for the facility fee of your visit. These bills will be mailed separately and may not be received at the same time. If you have any questions or need assistance regarding your charges, please call the phone number listed on the bill.

Hospital services will be billed by Elmhurst Memorial Hospital. Please ask your insurance company about how they cover bills for hospital-based clinics.
Insured patients: Both of the bills that you receive are subject to the terms, conditions and exclusions of your individual insurance policy. The bills may be subject to separate co-pays, deductibles and/or coinsurance, as defined by your insurance policy. Please contact your insurance company for detailed information regarding specific coverage.
Medicare patients: Both of the bills you receive may be subject to any applicable Medicare deductible and/or coinsurance provisions. With that in mind, you may be responsible for a portion of both bills. This amount is determined by Medicare and is based on the services provided.
