OFFICIAL RULES FOR THE 2018 EDWARD-ELMHURST HEALTH THE DRIVEN IDEA LAB PROMOTION

NO PURCHASE NECESSARY. The 2018 Edward-Elmhurst Health The Driven Idea Lab Promotion (“Promotion”) is a trade promotion open to all employees of Edward-Elmhurst Health and its wholly owned subsidiaries, medical staff members of Linden Oaks Hospital, Elmhurst Memorial Hospital and Edward Hospital, and volunteers at the foregoing hospitals who have registered with the Edward-Elmhurst Volunteer Office; provided all of the foregoing must be legal residents of the state of Illinois and have reached the age of majority (“Entrants”). All federal, state, and local laws apply. Void where prohibited. Promotion sponsored by the Edward-Elmhurst Health, 801 S. Washington Street, Naperville, IL 60540 (“Sponsor”). The Promotion begins at 12:00 a.m. central time on Oct. 17, 2018 and ends at 11:59 p.m. central time on Dec. 3, 2018 (the “Promotion Period”). This Promotion is a skills contest and the odds of winning depend upon the number of eligible entries received, the quality of the entries and the relative skill from the participants. All Promotion judges are prohibited from entering the Promotion.

The Promotion Period shall consist of the following periods:

  • Entry Period: Oct. 17, 2018 until Dec. 3, 2018
  • Initial Judging Period: Dec. 3, 2018 to Dec. 14, 2018
  • Finalist Presentation Period: Week of Feb. 11, 2019

To Enter: Entrants may enter the promotion by visiting the website at www.EEhealth.org/idealab and filling out the submission form to submit a proposal (“Entry”). Entries may be submitted into one of two categories:

  1. “Go For It” ideas – “Go For It” ideas are small scale improvements that can be quickly implemented without significant resources.
  2. “Large Scale Improvement” ideas – Larger ideas of either improvements or products which may require more significant resources, research and additional funding.

Entrants will determine which area to submit their Entry. Once submitted, the category of the Entry may not be changed by the Entrant. An Entry may only be submitted and considered for one category.

“Go For It” ideas may only be submitted by an individual Entrant. However, “Large Scale Improvement” ideas may be submitted by an individual Entrant or a group of up to three individuals (a “Team”) so long as the entire Team meets the eligibility requirements and each may be an Entrant. Each Team must designate a single member (the “Team Leader”) of the Team who will serve as the Team’s sole representative for communications and all other purposes in connection with the Promotion. Each Team member shall be considered a Entrant hereunder and must meet the residence and age eligibility requirements of the Promotion. All agreements, consents, representations and warranties made or given by a Team Leader herein or otherwise in connection with the Promotion will be binding upon all members of the pertinent Team as if made by each such member.

Sponsor is seeking entries which address one or more of the following:

  • Improvements that enhance patient safety and the quality of medical care.
  • Solutions to improve seamless communication among private physicians and patients.
  • Product and services which can directly impact the health of the patients Sponsor serves.
  • New or enhanced technology to improve workflows of the healthcare Sponsor delivers.

A valid Entry must comply with the following criteria:

  • All entries must be an original work created solely by the Entrant; no one other than the Entrant may have any rights of any kind in the Entry.
  • Entries may not contain or incorporate any third party content or infringe any third party rights.
  • Entries cannot contain inappropriate material, lewdness, obscenity, or the like, in Sponsor’s sole discretion.
  • All entries may not contain confidential information of the Entrant’s employer, or violate any other third party intellectual property rights or applicable laws.
  • Entries may not contain any personally identifiable information (other than the Entrant’s contact information), any protected health information or any other information governed by HIPAA or other privacy laws

Limit one (1) Entry per person or per Team during the Entry Period regardless of category entered. Individual persons can only be on one Team. Sponsor is not responsible for lost, late, incomplete, incorrect, damaged, or misdirected entries. Sponsor is further not responsible for problems of any kind related to the Promotion, whether mechanical, electronic, or human. PLEASE NOTE: By submitting an Entry, the contents/ideas in the Entry are licensed to Sponsor; see Section entitled “Confidential/Proprietary Information & License to Entry” below for more details.

Initial Judging Period: The Initial Judging Period will begin on Dec. 3, 2018 and end on Dec. 14, 2018. The Entries will be judged by a team of Sponsor’s staff during the Initial Judging Period (“First Round Judges”). The First Round Judges will consider each category of entries separately (either “Go For It” ideas or “Large Scale Improvement” ideas).

Small Scale Improvements and Selection of “Go For It” Winners: The Entries which fall into the category of small scale improvements will be judged on the following criteria (“Small Scale Improvement Criteria”) on a scale of 1-100 (per each Small Scale Improvement Criteria; total score equally weighted average).

  1. Strategic fit/alignment with Transformation PX™ (ideas advancing safe, seamless, and personal health care)
  2. Ease/speed of implementation
  3. Requires limited resources
  4. Overall likelihood of success/execution potential

The First Round Judges will select the top sixteen (16) Entries based on the Small Scale Improvement Criteria total scores to be deemed “Go For It” winners. In the event of a tie, “Go For It” winners will be selected by a panel vote (between the tied Entries) based on which Entry is likely to have the highest overall likelihood of success/execution potential.

Large Scale Improvements and Selection of Semi-Finalists: The Entries which fall into the category of Large Scale Improvements will be judged on the following criteria (“Large Scale Improvement Criteria”) on a scale of 1-100 (per each Large Scale Improvement Criteria; total score equally weighted average).

  1. Strategic fit/alignment with Transformation PX™ (ideas advancing safe, seamless, and personal health care)
  2. Competitive advantage/differentiation (existence of similar/competing solutions or ability to differentiate EEH)
  3. Size of impact/scalability (potential to impact on multiple patients/health systems)
  4. Limited resources/investment required
  5. Potential for strong financial return
  6. Overall likelihood of success/execution potential

The First Round Judges will select the top fifteen (15) Entries based on the Large Scale Improvement Criteria total scores to be deemed semi-finalists (“Semi-Finalists”). In the event of a tie, Semi-Finalists will be selected by a panel vote (between the tied Entries) based on which Entry is likely to have the highest overall likelihood of success/execution potential.

Notification of Semi-Finalists and “Go For It” Winners:
“Go For It” Winners will receive notification packages by e-mail on or about Dec. 15, 2018, which must be completed and returned to Sponsor by Dec. 17, 2018. The notification packages may include an affidavit of eligibility and publicity release which must be signed by Entrant and a document confirming the license to Sponsor set forth later in these rules applicable to all entries. In the event that the “Go For It” Winner does not return the notification package in the designated time period, Sponsor reserve the right void and disqualify the Entry and select an alternate “Go For It” Winner with the next highest total score.

Semi-Finalists will receive notification packages by e-mail on or about Dec. 18, 2018, which must be completed and returned to Sponsor by Dec. 28, 2018. The notification packages may include an affidavit of eligibility and publicity release which must be signed by Entrant or in the event the Semi-Finalist is a Team entry, the affidavit of eligibility and publicity release must be signed by all members of the Team. The notification packages will also include the dates of the mandatory workshops and final presentation. These events are mandatory and should the Semi-Finalists not be able to attend they will forfeit their position. If it is a Team entry, at a minimum, at least one member of the Team shall need to represent the Team at the events. In the event that Semi-Finalist does not return the notification package in the designated time period, Sponsor reserves the right void and disqualify the Entry and select an alternate Semi-Finalist with the next highest total score.

Semi-Finalists Round: All Semi-Finalists are invited to participate at two workshops to further examine their ideas and learn presentation guidelines, receive mentorship support, and strengthen their ideas from a business perspective. These workshops are mandatory and will take place during the weeks of January 7th and January 28th.

At the end of workshop #2, the Semi-Finalists will provide a pitch for the judge’s review. A panel of judges, selected by Sponsor, will then review and score each proposal based on the Large Scale Improvement Criteria listed above on a scale of 1-100 (per each Large Scale Improvement Criteria; total score equally weighted average). The Semi-Finalists with the top seven (7) scores will be deemed presenting finalists (“Presenting Finalists”). In the event of a tie, Presenting Finalists will be selected by a panel vote (between the tied Entries) based on which Entry is likely to have the highest overall likelihood of success/execution potential.

Presentation Period: The Presenting Finalists will each pitch their idea to a panel of judges during the week of Feb. 11, 2018 at Elmhurst Memorial Hospital or one of its facilities (“Final Presentation”). Unless otherwise agreed to separately, the Entrants are responsible for all expenses related to their attendance at the Final Presentation. Presenting Finalists may use note cards or any props including, but not limited to, PowerPoint slides, visual aids, toys, prototypes, clothing or accessories worn/used and called out to illustrate the idea, during their presentation. The First, Second, and Third Place Winners’ will be announced at the conclusion of the Final Presentation.

The panel of judges (“Final Judges”) will evaluate each Presenting Finalists based on the Large Scale Improvement Criteria on a scale of 1-100 (per each Large Scale Improvement Criteria; total score equally weighted average) to determine the first, second, and third place winners. In the event of a tie, first, second and third place winners will be selected by a panel vote of the judges (between the tied Entries) based on which Entry best embodies all of the criteria above.

Prize: The following prizes will be awarded:

  • “Go For It” Winner: $250 — 16 Winners
  • Semi-Finalist: $500 — 15 Winners
  • First Place Winner: $10,000
  • Second Place Winner: $7,500
  • Third Place Winner: $5,000

Total value of all prizes: $34,000. All prizes will be awarded, provided a sufficient number of eligible Entries are submitted in accordance with the Official Rules. No substitution or transfer of prizes, except at the sole discretion of Sponsor if advertised prize becomes unavailable.

General Terms and Conditions: The Promotion is governed by the laws of the United States and the State of Illinois. All federal, state and local laws and regulations apply. Prize will only be awarded and/or delivered to addresses within the United States. All federal, state and/or local taxes, fees, and surcharges are the sole responsibility of the prize winner. Failure to comply with the Official Rules will result in forfeiture of the prize.

Prize winners may be required to complete an affidavit or declaration of eligibility, waiver of liability and, where lawful, publicity release, which must be returned to Sponsor within seven days of receipt of notification package. If the effort to send a notification package to a winner is unsuccessful or if a winner fails to execute and return any required document within the required time period an alternate winner may be selected from remaining eligible Entrants.

If the prize is not claimed or is returned as undeliverable, then winner’s right to the prize shall be forfeited and the prize award will be void and an alternate winner may be selected from eligible Entrants. Sponsor is not responsible for late, lost, misdirected, incomplete, incorrect, misdirected entries or prize claims. Sponsor is also not responsible for incomplete, interrupted, or scrambled transmissions, unavailable web sites, networks, servers, or systems, or other connection or transmission problems, technical malfunctions of the computer, software, web site, or any combination thereof, problems associated with any virus or other damage caused to any participant’s system or other problems of any kind, whether mechanical, electronic, or human. Sponsor reserves the right to suspend and/or cancel the Promotion or change any element hereof, with or without notice, at any time, due to printing, computer or network errors, or other mistakes in the production or operation of the Promotion.

In the event of a dispute concerning who registered online to participate in the Promotion, the registration will be declared to have been made by the authorized account holder of the email address and who is defined as the natural person who is assigned to an email address by an internet provider, online service provider, or other organization (e.g., business, education institution, etc.) that is responsible for assigning email addresses for the domain associated with the email address. A potential winner may be requested to provide Sponsor with proof that the potential winner is the authorized account holder of the email address. If a dispute cannot be resolved to Sponsor’s satisfaction, the Entry will be deemed ineligible.

No correspondence regarding the Promotion will be entered into except with the “Go For It” Winners, Semi-Finalists, and Finalists and in response to requests for Winner’s Lists. All decisions of the judges and the Sponsor are final and binding on all matters relating to the Promotion. By entering, participants agree to be bound by the Official Rules, and release Sponsor and its agents and their respective affiliates, and each of their respective officers, directors, employees and agents from any and all liability, loss, or damage arising out of their participation in the Promotion or with respect to the awarding, receipt, possession, use and/or misuse of any prize. Winners are responsible for any applicable taxes on awarded prizes. Sponsor reserves the right to cancel or suspend the Promotion, or change any element thereof, with or without notice, at any time and for any reason. Sponsor reserves the right to disqualify in its sole discretion any person tampering with the Entry process or who violates these Official Rules in any way.

Confidential/Proprietary Information & License to Entry: Entrants may not use proprietary content and/or protected intellectual property that is not their own as part of the Entry. By submitting an Entry, Entrants are acknowledging that the Entry does not infringe on the protected rights of any third person or entity. Additionally, by entering the Promotion, any statements, claims, and information made as part of the Entry are no longer confidential.

By submitting an Entry, each Entrant grants Sponsor and its affiliates a perpetual, irrevocable, transferable, worldwide, royalty-free license to copy, display, distribute, make derivative works of, sublicense and otherwise use the Entry, any subsequent submissions or presentations as part of this Promotion, and all ideas, suggestions, improvements and solutions therein in any way and in any media without restriction or obligation to the Entrant. To the extent that Sponsor, its affiliates or any of its licensees utilize any ideas or content from an Entrant in development of any process, product or solution, Entrant acknowledges and agrees that Entrant shall have no ownership interest in or right to fees in connection with such process, product or solution. Publicity Release: By participating in the Promotion, the Entrant and, if applicable, all Team members (collectively, “Participating Parties”) irrevocably grant Sponsor the right to use the Participating Parties’ name, photograph, image, video, biographical information, and any other portion of your entry submission, in any and all media for any purpose, including, without limitation, advertising and promotional purposes and the Participating Parties hereby release Sponsor from any liability with respect thereto, except where prohibited. By entering this Promotion, you grant Sponsor the right to utilize the Participating Parties contact information provided by the Participating Parties in the entry to contact them for any reason. Completion of the entry form constitutes assent to these written terms.

Privacy Policy: Except as otherwise contemplated in these rules or in any publicity release, information collected by Sponsor in connection with this Promotion will be used by Sponsor in accordance with Sponsor’s online privacy policies and terms of use posted at www.eehealth.org/privacy-practices and www.eehealth.org/terms-of-use

Winners List: The winner’s list will be posted at www.EEhealth.org/idealab at least thirty days after the end of the Promotion.
